Welcome, current foster parents! We want you to know how much we appreciate all of your help with our dogs. It truly takes a wonderful, selfless person to bring a rescue dog into your home, not knowing what to expect. Sophia's Grace Foundation wouldn't exist, without the help and homes of our foster parents. This page will have most of the forms and information you may need when taking one of our pups into your home. Please feel free to browse around. Thank you all so much!
1. FOSTER DOG BIOGRAPHY FORM: Please fill out this form for each foster dog in your home. Complete it as soon as you bring them home, with as much information as you have. As you learn more about the dog, please return here to update the form with the new information you have.
2. VETERINARY CHECKLIST FOR FOSTER DOGS: Please print this Veterinary Checklist and bring it with you to your first veterinary appointment with your foster dog. Review it prior to arriving, so that you know what to make an appointment for and what is approved. Sophia's Grace Foundation pays the Veterinarian directly, so that you will not have to pay for treatments and get reimbursed. Before your first veterinary appointment, please get approval for the Veterinarian you will be using. Most Veterinarians will work with rescue organizations and offer discounts for services. We can provide 501(c)(3) documentation if needed. Sophia's Grace Foundation MUST approve your Veterinarian, before the first appointment.
